Portfolio of Evidence

The portfolio of evidence will eventually be completely electronic, allowing all ST doctors to upload important documents/certificates and will be a repository for all WPBAs. We are currently in negotiation to use the NHS e-portfolio, but in the meantime trainees can upload electronic versions of their own documents in the portfolio area of the website if they wish.

Whilst the NHS e-Portfolio is in development we suggest that every trainee compiles a ‘hard copy’ of their own portfolio of evidence, which should be divided into the following sections.

  1. Front page. Please use the Trainee information form to complete your demographics.
  2. CV – please keep an up to date copy of your CV
  3. WPBA forms – divided into the different sections of training i.e. ACCS, ST3 and ST4-6 (copies should be signed by the clinical supervisor to did your assessments). Please record these events on the record of WPBAs provided.
  4. Structured training reports – divided into the different sections of the training programme i.e. ACCS, ST3 and ST4-6. All StRs need to be copied to the ES.
  5. Notes from Meetings with Educational Supervisors (ES) or Clinical Tutors (CT). These should be clearly typed up, signed and dated by both trainee and trainer. Please use this table to record the dates of your meetings
  6. Personal development plan (PDP). Formulate your PDP before you meet with your ES and agree the outcomes and how these will be measure. Use this PDP document to record your agreements CEM - Learning portfolio - Personal development plan
  7. Audit and Clinical Governance activity– please keep a copy of any audit presentation and/or clinical governance activity that you have been involved in. If this involved a presentation to a group try to include a feedback form on the quality of your presentation, together with any reflective learning points. Please record the activity in the record of audit and clinical governance activity form below.
  8. Certificates – please keep an up to date section on your current life support certification and any other short courses that you have attended. If this involved any teaching please include relevant feedback forms.
  9. Educational Achievements – please include a table of your recent educational commitments with any feedback forms. Please use this record of educational activity table to record these events
  10. Research – If you have completed any research or published any articles this should be included in this section. Please note that academic trainees will have a separate system for documentation of research activity. Please use this table to record the overall events.
  11. Management activity. Please include a table of your recent management activity. This may involve rota writing (include over how long), complaint answering, involvement in departmental meetings, change management projects, equipment acquisition, process management and dealing with other agencies. If you have had any feedback on your contribution please include it in this section. Please use this record of management activity to record you overall activity.
  12. Clinical case log book. In this section try and record anonymised patients whose care you have been involved. Divide this into ACCS, ST3 and ST4-6. Try to demonstrate that a wide breadth of clinical case mix has been seen together with reflective learning points.
  13. Absence from training programme. It is essential that you record your annual and study leave in the table provided. This will help distribute your leave evenly throughout the training period. You also need to record any time off with sickness or for any other reason such as an OOPE.
